Club Request for a COI

Certificate of Insurance (COI)

YES-Athletics 365™ Clubs can request a Certificate of (Liability) Insurance (COI) when required from the property owner of club practice areas and venues where the club is hosting competition.  There is no additional cost for 365 Clubs to request a COI for their practice locations and venues for competition.

Please allow two (2) weeks from the date your request was submitted to receive a COI.  (You will receive the COI by email only.)

Each request is made from the Coordinator's Dashboard.
